All admission forms for the 2026/27 school year must be completed
and returned ASAP; we must have them before your child can attend class.

• Annual Health Form

• Certificate of Immunization Card

• Emergency Contact Form

• Teach Parent Form

Please download fillable PDFs and (preferably) type answers via computer, or hand write clearly and print out each form so they can be added to your physical file. The state of Colorado requires us to have all forms printed and on-hand in each child’s file.
